About the Role

Gentiva Hospice is seeking a dedicated and compassionate Patient Care Manager to join our team. As a key member of our hospice team, you will be responsible for the day-to-day operations of your assigned hospice site, ensuring the provision of high-quality care to patients and their families.

Key Responsibilities
  • Administer the clinical aspects of the hospice program, ensuring compliance with regulatory guidelines and managed care principles.
  • Manage and oversee clinical and administrative functions, including recordkeeping and staff supervision.
  • Respond to phone referrals and inquiries in a timely, professional, and compassionate manner.
  • Participate in on-call nursing schedules and provide direct skilled bedside nursing care in patients' homes.
Requirements
  • Registered Nurse with a minimum of Associate's Degree in Nursing required from an accredited school of nursing.
  • Three years' experience in direct patient care in a clinical setting required.
  • Minimum one year of management/supervisory experience required.
  • Two or more years' experience in hospice or home care required.
  • Current license to practice nursing in the state of requested employment.
