We are seeking a highly skilled and compassionate Clinical Social Worker to join our team at Saban Community Clinic. As a key member of our Behavioral Health Department, you will play a vital role in providing high-quality mental health services to individuals experiencing homelessness.

Responsibilities:
  • Assess, diagnose, and treat patients' symptoms and dysfunctional behaviors.
  • Provide psychosocial rehabilitation treatment, individual and group psychotherapy, and case management services.
  • Develop effective treatment plans in collaboration with other care team members and clients.
  • Facilitate independence and maintenance of improved physical and mental health through individualized support and referrals.
  • Provide leadership and consultation on mentally ill, homeless, and supportive housing best practice models.
Requirements:
  • Masters' degree in Social Work.
  • Licensed as an LCSW.
  • Minimum three years of providing mental health services to homeless, mentally ill, substance-addicted adult populations.
  • Prior experience working within a community mental health or primary care setting preferred.
  • Able to work in a harm reduction model with clients who have multiple barriers.
