We are seeking a compassionate and skilled Licensed Practical Nurse to join our team at The Reservoir Center for Health & Rehabilitation.
Job SummaryThe Licensed Practical Nurse will play a vital role in ensuring the well-being and comfort of our residents. This includes providing direct nursing care, administering medications, collaborating with healthcare professionals, and overseeing the implementation of care plans.
Key Responsibilities- Administer medications and treatments as prescribed by healthcare providers
- Monitor and record patient vital signs and medical information
- Collaborate with the interdisciplinary team to develop and implement care plans
- Provide wound care and other nursing interventions as needed
- Supervise and support Certified Nurse Assistants (CNAs) in delivering care
- Ensure compliance with all regulatory standards and protocols

- Valid state license (LPN)
