About the Role
  • Provide one-on-one patient care in a home setting, working with a diverse patient population.
  • Collaborate with our team of specialized nurses, rehabilitation therapists, social workers, and administrative staff to deliver personalized patient care.
  • Participate in on-call coverage rotations and contribute to process improvement initiatives within the agency.
Responsibilities
  1. Coordinate case management activities within a multidisciplinary healthcare team to ensure appropriate delivery of services to patients.
  2. Initiate, coordinate, and ensure ongoing communication with physicians and other healthcare team members.
  3. Attend and participate in patient care conferences, team meetings, and other related meetings.
  4. Identify, develop, and participate in process improvement opportunities to enhance the quality of services provided.
  5. Provide direct patient care, evaluate outcomes, and adjust the nursing care process as indicated to ensure optimal patient care.
  6. Establish the plan of care in collaboration with the physician, patient, and UM Professional in accordance with physician orders and patient needs.
  7. Complete documentation, including visit assessments, OASIS, 485, and verbal orders, accurately and promptly on the laptop device or in written documentation.
Requirements
  • Zero to two years of experience preferred.
  • Six months of nursing experience in an acute care or skilled care facility, home health, or hospice care preferred.
  • BSN preferred.
  • Successful completion of Home Health/Hospice Orientation upon hire.
Licensure, Certifications, and Clearances
  • Automotive Insurance.
  • Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR).
  • Driver's License.
  • Registered Nurse (RN) or Temporary Practice Permit (TPP).
  • Act 33.
  • Act 34.
  • OAPSA.

Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state.
