UPMC Home Health is seeking a full-time Resource RN to join our team in Mercer and Crawford County. This role involves working Monday to Friday with on-call rotations, providing direct patient care and collaborating with our multidisciplinary team.
About the Role- Provide one-on-one patient care in a home setting, working with a diverse patient population.
- Collaborate with our team of specialized nurses, rehabilitation therapists, social workers, and administrative staff to deliver personalized patient care.
- Participate in on-call coverage rotations and contribute to process improvement initiatives within the agency.
- Coordinate case management activities within a multidisciplinary healthcare team to ensure appropriate delivery of services to patients.
- Initiate, coordinate, and ensure ongoing communication with physicians and other healthcare team members.
- Attend and participate in patient care conferences, team meetings, and other related meetings.
- Identify, develop, and participate in process improvement opportunities to enhance the quality of services provided.
- Provide direct patient care, evaluate outcomes, and adjust the nursing care process as indicated to ensure optimal patient care.
- Establish the plan of care in collaboration with the physician, patient, and UM Professional in accordance with physician orders and patient needs.
- Complete documentation, including visit assessments, OASIS, 485, and verbal orders, accurately and promptly on the laptop device or in written documentation.
- Zero to two years of experience preferred.
- Six months of nursing experience in an acute care or skilled care facility, home health, or hospice care preferred.
- BSN preferred.
- Successful completion of Home Health/Hospice Orientation upon hire.
- Automotive Insurance.
- Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR).
- Driver's License.
- Registered Nurse (RN) or Temporary Practice Permit (TPP).
- Act 33.
- Act 34.
- OAPSA.
Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state.
