We are seeking a skilled Licensed Practical Nurse to join our team at Developmental Services, Inc. in Bloomington. The estimated salary for this position is $60,000-$70,000 per year, depending on experience.
Responsibilities- Develop and oversee the maintenance and routine monitoring of Waiver participants Wellness Support plans per the Wellness Standard guidelines.
- Complete face-to-face assessments in accordance with the Wellness Tier Guidelines. Consults as determined by current healthcare needs.
- Complete Wellness Quarterly reports.
- Follow treatment and medication plans for clients as recommended by a physician.
- Assist in the scheduling of medical appointments. Attend medical or specialty appointments when necessary.
- Maintain accurate medical files for clients. Prepare and maintain other records pertaining to medical information.
- Provide direct care and first aid to clients.
- Plan and present Infectious Disease Training for employees as necessary.
- Maintain great communication and professional relationships with clients, family members, and co-workers.
- Monitor, order, and stock medical supplies.
- Coordinate/administer physician-ordered injections/inoculations as needed.
- Review of Medication Administration record/Physicians' orders.
- Coordinate/conduct medical in-services as needed.
- Must meet physical requirements. Physical requirements may include but not limited to bending, reaching, stretching, lifting 25-50 pounds, running, walking, squatting, assistance with physical transfer, handling physical aggression as defined in CPI training.
- Teach agency Med Core A & B classes as needed.
