We are seeking a compassionate and skilled Registered Nurse to join our Hospice team at St. David's Hospice & Family Care. As a Hospice RN, you will play a vital role in providing high-quality patient care and supporting families during their most challenging times.
About Our TeamAt St. David's Hospice & Family Care, we pride ourselves on delivering exceptional care and support to our patients and their loved ones. Our interdisciplinary team includes physicians, nurses, therapists, hospice aides, and social workers who work together to provide holistic care and address the unique needs of each individual.
Responsibilities- Provide direct patient care in accordance with physician orders and established standards of practice
- Develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients and their families
- Collaborate with the healthcare team to ensure seamless coordination of care and effective communication
- Participate in ongoing education and training to stay current with best practices and evidence-based care
- Current licensure as a Registered Nurse (RN) in the state of employment
- Minimum 1 year of experience in hospice or palliative care
- Associate Degree in Nursing or higher
