We are seeking an experienced Palliative Care Nurse Specialist to join our team at Liberty Hospice.

Key Responsibilities
- Apply theoretical knowledge of nursing practices to deliver high-quality patient care.
- Coordinate physical care and document patient information in accordance with reimbursement guidelines.
- Cultivate effective communication with patients, families, physicians, and interdisciplinary teams to ensure seamless care delivery.
- Maintain open dialogue with agency personnel to optimize patient outcomes.
- Participate in scheduled IDT meetings and quality reviews to drive continuous improvement.
Requirements
- Holding current RN licensure and a degree in nursing from an accredited institution, along with two years of clinical experience and one year of hospice nursing experience preferred.
- Demonstrating expertise in home health and hospice care frameworks, functions, and organizational structures, as well as the nurse's role in patient care teams.
- Working knowledge of nursing principles and practices according to state laws, agency policies, and reimbursement guidelines.
- Completion of 12 hours of continuing education annually and CPR certification.
- Flexibility for travel and weekend/evening call assignments as needed.
Estimated Salary: $65,000 - $80,000 per annum
