We are seeking a highly skilled Registered Nurse Care Manager to join our team at AdventHealth. As a key member of our care coordination team, you will play a vital role in ensuring seamless transitions of care for our patients.
Key Responsibilities- Conduct thorough patient evaluations to identify post-hospital needs and develop effective transition of care plans.
- Collaborate with the interdisciplinary team to ensure efficient and cost-effective care through appropriate resource monitoring and clinical care escalations.
- Communicate daily with the healthcare team during multidisciplinary rounds to facilitate high-quality patient progression of care and transitions plans.
- Evaluate patient readmission risk scores and coordinate readmission mitigation interventions to prevent unnecessary hospital readmissions.
- ADN, RN with a minimum of two years of hospital nursing experience.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Knowledge of post-hospital care services and resources, including home health, infusion services, durable medical equipment, and community-based organizations.
