Nurse Care Coordinator

Job Summary:
CareOne is seeking a skilled Registered Nurse to join our team as a Long Term Care Specialist. In this role, you will be responsible for providing high-quality nursing care to our residents, while also coordinating with the interdisciplinary team to ensure seamless care delivery.
Key Responsibilities:
- Provide direct nursing care to assigned residents, including assessment, planning, implementation, and evaluation of care.
- Coordinate with the interdisciplinary team to develop and implement individualized care plans.
- Collaborate with other healthcare professionals to ensure continuity of care and optimal resident outcomes.
- Document resident care and progress in accordance with facility policies and procedures.
- Participate in quality improvement initiatives to enhance resident care and satisfaction.
Requirements:
- Associate's Degree in Nursing from an accredited program.
- Current licensure as a Registered Nurse in the state of practice.
- 1 year of experience in long-term care or a related field.
