Role Summary:

The Client Support Specialist is responsible for fostering a professional rapport with clients, primarily focusing on conducting comprehensive health assessments.

Key Responsibilities:

  • Engage with the Integrated Care Coordination Team as necessary.
  • Facilitate logistics to support the objectives of care plans and interventions, addressing immediate client needs.
  • Confirm eligibility, prior enrollment history, demographics, and current health status of each client before outreach.
  • Conduct assessments by collecting information from clients, families, providers, and other relevant parties, as directed by the client or their authorized representative.
  • Assist in the formulation and execution of care plans, providing feedback to the Care Coordinator based on assessment results when required.
  • Support the delivery of health education, wellness resources, and coaching as appropriate.
  • Maintain accurate documentation in accordance with the protocols and guidelines of the Care Management program.
  • Approach each interaction with clients by considering their unique circumstances and how best to assist them, ensuring they feel informed, empowered, and supported.
  • Identify opportunities for process improvements to enhance the client experience and streamline assessment and follow-up procedures.
  • Perform additional duties as assigned.

Qualifications:

  • High School Diploma or General Education Diploma (GED) is mandatory.
  • A minimum of two (2) years of experience in volunteer or paid roles within community settings, particularly with at-risk populations, is preferred.

Skills and Competencies:

  • Proficient in Microsoft Office Suite, including Outlook, Word, and Excel.
  • Experience and sensitivity in working with diverse cultures.
  • Strong interpersonal skills.
  • Ability to work autonomously as well as collaboratively within a team.
  • Capable of identifying issues and opportunities and effectively communicating them to management.
  • Basic understanding of local, state, and federal healthcare regulations and company policies related to case management.
  • Exhibit compassion, support, and collaboration with clients and their families.
  • Self-driven and curious.
  • Comfortable asking relevant questions.
