Job SummaryWe are seeking a highly skilled Hospice Registered Nurse to join our team. As a key member of our healthcare team, you will be responsible for planning, coordinating, providing, and documenting care for a geographically assigned caseload of patients from day of admission through day of discharge.
You will utilize physicians' orders and professional skills to develop and implement a home health plan that meets each patient's specific needs and complies with accrediting organization standards, federal and state regulations, reimbursement guidelines as well as HHA policies and procedures.
Responsibilities- Perform admission assessment within 24-48 hours of referral or patient return to residence per agency guidelines.
- Develop a written plan of care for each patient.
- Collect accurate OASIS data at time appropriate periods.
- Assess patient's condition, initiate plan of care, re-evaluate and update as necessary.
- Able to adequately assess and reassess pain. Utilize appropriate pain management techniques. Educate patient and family regarding pain management.
- Perform patient care responsibilities considering needs specific to standard of care for patient's age.
- Demonstrate knowledge of medications and their correct administration based on age of patient and patient's clinical position.
- Follow seven medication rights and reduce potential for medication errors.
- Provide skilled nursing care.
- Obtain all necessary physician orders and collaborate with physician as patient's needs dictate.
- Perform all aspects of patient care in environment that optimizes patient safety and reduces likelihood of medical/healthcare errors.
- Support and maintain culture of safety and quality.
- Initiate case conferences to discuss multidisciplinary team responsibilities, patient progress, plans for continued care, new problems, etc.
- Notify Intake Coordinator of patients who need to be seen on weekends or holidays and submit updated nursing care plan to weekend/holiday nurse.
- Follow up with obtaining lab results and notifying physician of any significant results.
- Establish patient schedule and indicate which patients may be seen by part-time RN when caseload prohibits Case Manager from seeing all scheduled patients.
- Re-evaluate need for continued care on ongoing basis, initiate recertification as necessary.
- Complete patient documentation on time.
- Attend in-services and staff meetings as needed.
- Complete assigned training on time.
- Complete annual education requirements.
- Always maintain patient confidentiality.
- Perform other duties as assigned.
