Patient Advocate Coordinator

The Patient Advocate Coordinator provides critical support to patients navigating their treatment within Cone Health. This role serves as a patient's advocate, working with the primary care team to improve quality of care and health outcomes.
Key Responsibilities:
- Provides scheduling assistance to patients, ensuring timely arrival at appointments and enhancing office-wide efficiency.
- Arranges transportation for patients who cannot attend appointments without support, promoting patient and community health.
- Serves as a champion for patients, guaranteeing the primary care team provides optimal care.
- Partners with the primary care team to connect patients with community and network-based health support systems.
- Communicates effectively with patients, families, and providers to increase understanding and ensure a positive experience.
Requirements
- High School Diploma or equivalent is required.
- At least 2 years of experience in Care Management or equivalent university studies coursework is necessary.
We offer a competitive salary range: $45,000 - $60,000 per year, depending on qualifications and experience.
