About the Role

VNA Health Group is seeking a skilled Physical Therapist to join our team in providing high-quality care to patients in their homes or alternate community settings.

Responsibilities
  • Conduct thorough assessments and evaluations of patients to identify physical therapy needs and develop personalized plans of action in collaboration with physicians.
  • Provide evidence-based physical therapy interventions to patients, focusing on improving mobility, strength, and functional abilities.
  • Document all changes in patient condition, update care plans, and track outcomes to ensure optimal care delivery.
  • Identify counseling needs and develop teaching strategies to empower patients and their families with knowledge and skills to manage their conditions.
  • Monitor the quality and appropriateness of services provided, adhering to agency infection control and safety policies.
  • Meet agency requirements for absenteeism and appearance, ensuring a professional and respectful work environment.
Requirements
  • Current Physical Therapy licensure in New Jersey is mandatory.
  • 2-3 years of prior clinical experience and community-based experience are preferred.
  • Strong computer skills and effective communication and interpersonal skills are essential.
  • A valid New Jersey driver's license is required.
Working Conditions/Physical Demands

This role involves working indoors and outdoors, driving, lifting, sitting, standing, walking, fine motor hand movement, and using a phone and tablet for documentation.
