Job Summary:The Skill Builder will work closely with our clinical team to develop and implement treatment plans that address the unique needs of each patient. Key Responsibilities:- Assist in the development, implementation, and revision of individual treatment plans to ensure that services provided are specified in the treatment plan and progress toward treatment goals is monitored.
- Provide individual and group skills training and development to assist patients and/or families in developing skills such as self-care, daily life management, interpersonal communication, and problem-solving skills.
- Work collaboratively with the clinical team to ensure coordination of integrated care for patients, including identification of health issues, compliance with treatment, and effective patient-to-provider communication.
- Complete CANS/ANSA re-assessments and provide consultation to staff on specific cases involving coordination of care.
- Maintain positive working relationships with staff of other agencies and institutions, and act as a liaison between patients and agencies and/or institutions.
- High school diploma or equivalent with 2 years of paid behavioral healthcare experience or equivalent college coursework toward a human services or health-related degree required at minimum.
- Associate or Bachelor's degree preferred.
- A valid driver's license with a driving record meeting Meridian's driving policy, reliable transportation, and proof of auto insurance.
- A commitment to providing individual attention to help clients achieve mental, emotional, and physical well-being.
