Meridian Health Services is seeking a skilled and compassionate Skill Builder to join our team. As a key member of our healthcare team, you will play a vital role in providing individualized support and guidance to patients and their families.
Job Summary:The Skill Builder will work closely with our clinical team to develop and implement treatment plans that address the unique needs of each patient. This may include providing skills training and development, case management services, and coordination of care with other healthcare providers.
Key Responsibilities:- Assist in the development, implementation, and revision of individual treatment plans to ensure that services provided are specified in the treatment plan and progress toward treatment goals is monitored.
- Provide individual and group skills training and development to assist patients and/or families in developing skills such as self-care, daily life management, interpersonal communication, and problem-solving skills.
- Work collaboratively with the clinical team to ensure coordination of integrated care for patients, including identification of health issues, compliance with treatment, and effective patient-to-provider communication.
- Complete CANS/ANSA re-assessments and provide consultation to staff on specific cases involving coordination of care.
- Maintain positive working relationships with staff of other agencies and institutions, and act as a liaison between patients and agencies and/or institutions.
- High school diploma or equivalent with 2 years of paid behavioral healthcare experience or equivalent college coursework toward a human services or health-related degree required at minimum.
- Associate or Bachelor's degree preferred.
- A valid driver's license with a driving record meeting Meridian's driving policy, reliable transportation, and proof of auto insurance.
