We are seeking a highly skilled Pharmacist to join our team in a long-term care setting. The ideal candidate will have a strong background in pharmacy practice and a passion for delivering exceptional patient care.
Key Responsibilities:
- Evaluate physician medication orders for appropriateness and make recommendations for therapy changes as needed.
- Maintain accurate and up-to-date patient/resident databases, including current and past medical history, clinical and laboratory data, and concurrent medication.
