Case Manager RN

The Mount Sinai Health System is seeking a skilled Case Manager RN to join our team at Mount Sinai Morningside. As a Case Manager RN, you will play a critical role in ensuring the optimal care and management of our patients.
Key Responsibilities:- Conduct thorough assessments of patients to identify their clinical, psychosocial, and functional needs.
- Develop and implement individualized care plans to address patient needs and promote optimal outcomes.
- Collaborate with interdisciplinary teams, including physicians, social workers, and other healthcare professionals, to ensure seamless care coordination.
- Monitor patient progress and make adjustments to care plans as needed.
- Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and optimal care.
- Bachelor's degree in Nursing; Master's degree preferred.
- Previous experience in home care, long-term care, or utilization review preferred.
- Discharge Planner or Case Manager experience preferred.
- Minimum of 3-5 years' experience as a RN in an acute care setting.
- Current NYS RN nursing license.
- CCM certification preferred; re-credentialing expected.
- PRI Certification (to be obtained within 1 year).
