Case Manager RN

We are seeking a skilled RN Case Manager to join our team at ChristianaCare. As a RN Case Manager, you will play a critical role in coordinating patient care and driving patient progression to establish a discharge plan.
About the Role:The RN Case Manager will be responsible for:
- Coordinating patient care and developing a discharge plan
- Collaborating with the interdisciplinary team to ensure a multidisciplinary plan of care is in place
- Identifying system issues that serve as barriers to care and participating in the development and implementation of strategies to remove barriers and facilitate patient progression

Requirements:To be successful in this role, you will need:
- An active RN license in DE or Compact State
- BSN required
- RN with a minimum of 2 years of medical surgical nursing in an acute care hospital
- A minimum of 1 year of Care Management experience
- BLS preferred
- Case Management Certification (CCM) required within 18 months of eligibility
