Job Summary

We are seeking a skilled Licensed Practical Nurse-Outpatient to join our team at WellSpan Health. As a key member of our healthcare team, you will provide high-quality patient care and support in an outpatient setting.

Key Responsibilities
  • Obtain patient history and gather information to inform care decisions
  • Perform specialized procedures, examinations, and tests as ordered and aligned with the Ambulatory Clinical Scope Document
  • Ensure patient care areas are adequately stocked with unexpired supplies and medications
  • Relay information and facilitate communication between providers and patients
  • Evaluate patient health status and direct patients to the most appropriate level of care
  • Track and review diagnostic test results to ensure timely notification and follow-up
  • Provide patient education and counseling on healthy lifestyle changes and disease management
  • Assist in the management of patient care between visits and provide support for clinical staff
Requirements
  • Diploma or Associate's degree in LPN program from an accredited institution
  • 1 year of clinical nursing experience
  • Current LPN licensure in Pennsylvania or multi-state licensure
  • Basic Life Support certification
