Job Summary:

We are seeking a skilled and compassionate Registered Nurse to join our team at Kaiser Permanente's Transitional Care Unit in San Diego. As a leader in patient care, you will be responsible for providing high-quality nursing services to our patients, utilizing the nursing process to ensure optimal outcomes.

Key Responsibilities:
  • Develop and implement individualized plans of care that reflect assessment, planning, implementing, and evaluating patient outcomes.
  • Collaborate with the healthcare team to ensure seamless care transitions and effective communication.
  • Provide education and support to patients and families to promote health and wellness.
  • Participate in performance improvement activities to enhance patient care and safety.
  • Lead by example, demonstrating a commitment to excellence and a passion for delivering exceptional patient care.
Requirements:
  • 1 year of recent experience in a Critical Care, DOU, or SDU setting.
  • Registered Nurse License (California) and Basic Life Support certification.
  • Advanced Cardiac Life Support certification preferred.
