Job Summary:
We are seeking an experienced Nurse Practitioner to join our team. The successful candidate will be responsible for direct patient care, education, and health planning. This includes assessing patients' needs, developing individualized care plans, and collaborating with other healthcare professionals to ensure optimal outcomes.
Responsibilities:
- Conduct thorough physical assessments and develop personalized care plans
- Coordinate services with other disciplines and patient insurance plans
- Advocate for patients in arranging medically necessary services
- Communicate effectively with community resources to secure assistance for patients in need
- Refer patients to specialty services when necessary
- Consult with collaborating physicians and healthcare team members to establish care plans
- Maintain continuity of care through regular patient follow-up
- Participate in quality assurance processes to ensure optimal care delivery
- Provide comprehensive case management services, emphasizing health promotion and disease prevention
- Monitor patient outcomes according to practice protocols
- Collaborate with staff nurses and administrators in assessment and clinical decision-making for patient care planning
- Act as a resource person for nursing staff
- Conduct educational sessions on practice issues
Qualifications:
- Minimum 1 year experience as NP
- Graduated from an approved Nurse Practitioner Program
- Licensed as a Nurse Practitioner in New York State
- Basic CPR (BCLS) certification required
- Specialty Certification Preferred
- Bilingual in English and Spanish or Creole preferred
Compensation and Benefits:
We offer a competitive salary range of $85,000-$110,000 per annum, depending on experience. Additional benefits include health and dental insurance, paid time off, 401-K with company match, paid holidays, employee discounts, and more.
