We are seeking a highly skilled and compassionate Outpatient Therapist to join our team at Communicare. As a key member of our treatment team, you will provide evidence-based services to individuals in their residences or other community settings.
Responsibilities- Provide high-quality, patient-centered care while maintaining fidelity to established treatment protocols.
- Collaborate with clients, families, and other healthcare professionals to develop and implement individualized treatment plans.
- Arrange for medication evaluation, management, and monitoring to ensure optimal client outcomes.
- Participate in regular clinical supervision to maintain the highest standards of mental health care.
- Meet all Communicare, Department of Mental Health, and Division of Medicaid standards and policies regarding patient care and documentation.
- Provide timely services and documentation as directed, meeting productivity requirements and working efficiently in our Electronic Health Record system.
- Attend and conduct meetings, training sessions, and other events as assigned, including crisis intervention and suicide prevention training.
- Assist in collecting and maintaining data as needed or required.
- Serve as a member of our treatment team, working collaboratively with other programs to ensure continuity of care.
- Provide rapid response crisis services, intakes, and therapy services as directed by your supervisor.
- Assess home and family situations to ensure client needs are met and work directly with families to ensure participation in treatment.
- Participate in conferences and meetings with referral sources and other outside agencies pertinent to individual treatment plans.
- Master's Degree and licensure or certification in counseling, psychology, social work, marriage and family therapy, or other related behavioral health care fields preferred.
- Pass a Communicare background check and drug screen, as we maintain a drug-free workplace.
- Ability to perform the responsibilities of the position as described above.
- Ability to work independently and relate positively to individuals, families, and staff members.
- Must have reliable transportation and the ability to travel throughout the catchment area and to trainings and meetings in other locations as needed.
