Outpatient Therapist

Job Summary

We are seeking a highly skilled and compassionate Outpatient Therapist to join our team at Communicare. As a key member of our treatment team, you will provide evidence-based services to individuals in their residences or community settings, promoting continuity of care and supporting their mental health needs.

Responsibilities
  • Provide high-quality, patient-centered services while maintaining fidelity to established treatment plans.
  • Collaborate with clients, families, and other healthcare professionals to ensure comprehensive care.
  • Develop and maintain an active caseload, adhering to Communicare's standards and policies.
  • Meet productivity requirements and provide timely services and documentation.
  • Work efficiently in Communicare's Electronic Health Record system and participate in training and meetings as assigned.
  • Assess home and family situations to inform treatment plans and work closely with families to ensure their participation.
  • Participate in conferences and meetings with referral sources and other outside agencies to ensure continuity of care.
  • Establish and maintain referral bases within the community.
Requirements
  • Master's Degree and licensure or certification in counseling, psychology, social work, or a related field.
  • Ability to perform the responsibilities of the position and work independently.
  • Reliable transportation and a valid driver's license, automobile insurance, and an insurable driving record.
  • Pass a Communicare background check and drug screen.
Compensation

$37,000 hourly wage.
