Job Title: Clinical Case Manager

Felton Institute is seeking a highly skilled Clinical Case Manager to join our Older Adult Service Team (OAST) in Alameda County. As a Clinical Case Manager, you will work collaboratively with a multidisciplinary team to provide comprehensive case management services to older adults with severe mental illness and co-occurring disorders.

Job Summary:

The Clinical Case Manager will assess and identify the needs of clients, connect them with appropriate services, and advocate on their behalf. This role requires strong clinical knowledge, experience working with older adults, and excellent communication and interpersonal skills.

Responsibilities:
  • Perform intakes for older adults with severe mental illness in a Felton clinic, in the client's home, or in an appropriate field-based location.
  • Provide case management services, including evaluations, treatment plan development, and assisting clients in accessing housing, financial, medical, and other needed services.
  • Provide therapy and other mental health interventions, as well as crisis intervention and management to prevent hospitalization.
  • Maintain accurate and detailed documentation, including clinical records and Medi-Cal billing requirements.
  • Participate in team meetings and case conferences to discuss clinical issues and exchange information and ideas.
  • Commit to ongoing professional development in the geriatric specialization of mental health services.
Requirements:
  • A minimum of two years of case management and mental health treatment experience working with at-risk older adults, including co-occurring disorders.
  • Masters degree or higher in Social Work, Counseling, or Psychology, and active registration with the BBS - AMFT, APCC, ASW, LMFT, LPCC, LCSW, highly preferred.
  • Comprehensive range of mental health and substance abuse counseling, assessment, and crisis intervention experience.
  • Ability to work cooperatively on a multi-disciplinary team and with community partners and collaborators.
  • Knowledge of DSM V-TR and experience with Medi-Cal billing requirements.
  • Valid CA Driver's License and vehicle.
