About the Role

AHN Healthcare@Home is seeking a compassionate and skilled Home Health Registered Nurse to join our team. As a Home Health RN, you will play a vital role in delivering high-quality patient care in the comfort of patients' homes.

Key Responsibilities
  • Lead and collaborate with the care team to develop and implement patient care plans
  • Provide management and delivery of patient care plans, ensuring the highest level of quality and patient satisfaction
  • Work closely with patients, families, and caregivers to understand their needs and preferences
  • Collaborate with the interdisciplinary team to ensure seamless care transitions and coordination
  • Document patient progress and outcomes in a timely and accurate manner

Requirements
  • RN license in the state you work
  • Graduate from an approved school of practical nursing
  • One year of RN experience and the clinical competence to deliver quality patient care
  • Current driver's license and ability to spend ~20% of your day driving to/from patient locations
  • A commitment to consistently meet critical deadlines for charting
  • The skills needed to self-manage your time and schedule
  • Demonstrated experience with tablets, mobile phones, and EMR software
