Job Summary

We are seeking a skilled Veteran's Case Manager to join our team at Delta-T Group Inc. in Philadelphia, PA. As a case manager, you will work with veterans in the Philadelphia area who are dealing with various challenges, including homelessness and mental health issues.

Key Responsibilities
  • Engage and assist veterans in receiving accurate services on a case-by-case basis.
  • Maintain a caseload and ensure each individual receives the necessary support.
  • Help veterans find housing, reestablish connections with family, and access employment or medical/psychiatric care.
Requirements
  • Bachelor's Degree in Human Services, Psychology, Sociology, or a related field.
  • At least 1 year of experience in case management.
  • Ability to complete and pass clearances, including FBI, Child Abuse, PA State Background Check, and Act 126.
  • Current medical/health documents, including CPR Certification, First Aid Level 2 Certification, and physical examination.
