As a Home Health LPN, you will be responsible for providing skilled nursing visits to clients in the comfort of their own homes. You will work closely with other healthcare professionals to develop and implement individualized plans of care that meet the unique needs of each client.
Key Responsibilities:
- Assist in the development, review, and revision of the plan of care.
- Provide nursing care in accordance with physician orders and individual plans of care.
- Observe, record, and report any changes in client condition to appropriate personnel and/or physician.
- Provide ongoing instruction and supervision to clients, family members, or other healthcare team members.
Requirements:
- Kansas or Compact Nursing License.
- Home Health experience required.
- Driver's License and Vehicle with Current Auto Insurance.
- Strong organizational and self-management skills.
- Excellent communication skills.
