Job Title: Physical Therapist

We are seeking a skilled Physical Therapist to join our team at Kaleida Health. As a Physical Therapist, you will be responsible for planning, implementing, and evaluating physical therapy services in the home care setting.

Job Summary:

The successful candidate will have a Bachelor's Degree in Physical Therapy and a valid New York State license to practice. One year of experience in Physical Therapy is preferred. The ideal candidate will have strong communication skills, both verbal and written, and be able to work effectively with patients and other healthcare professionals.

Responsibilities:
  • Plan and implement physical therapy services in the home care setting
  • Evaluate patient progress and adjust treatment plans as needed
  • Communicate effectively with patients, families, and healthcare professionals
  • Delegate tasks to Physical Therapy assistants and Home Health Aides as needed
Requirements:
  • Bachelor's Degree in Physical Therapy
  • Valid New York State license to practice
  • One year of experience in Physical Therapy (preferred)
