Preventative Health Care Coordinator

The Preventative Health Navigator is a registered nurse with expertise in preventative health who provides personalized support to patients, families, and caregivers to overcome barriers to care. Utilizing the nursing process, the navigator offers education and resources to facilitate informed decision-making and timely access to quality health screenings, surveillance, and care coordination services.
Key Responsibilities- Coordinate Care: Performs the nursing process (assessment, diagnosis, identification of outcomes, planning, implementation, and evaluation) to deliver evidence-based practice, ensuring individualized care and coordination through effective partnerships.
- Improve Patient Care Timeliness: Collaborates with program leaders and administration to evaluate data from community needs assessments, identifying areas for improvement that impact the patient navigation process and program.
- Provide Relationship-Based Care: Offers patient-centered care that adapts service delivery to reflect an understanding of diversity, collaborating with the population health team and navigation team to coordinate high-level care for all patients.
- Education: Associate's degree in nursing from an accredited/approved school of nursing. Bachelor's degree in nursing (BSN) from an accredited school of nursing preferred. Master's degree in a healthcare-related field preferred.
- License and Certification: Basic Life Support (BLS) certification. Registered Nurse (RN) license with a single state or multi-state compact.
- Experience: Minimum 5 years of nursing practice. Oncology, pulmonary, thoracic, colorectal, or outpatient case management experience preferred.
- Communication: Strong interpersonal, collaborative, and customer service skills.
- Critical Thinking: Ability to function in a fast-paced environment, respond to emergencies, and make decisive decisions.
- Time Management: Excellent time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
