Job Title: Case Manager

We are seeking a skilled and compassionate Case Manager to join our team at Professional Case Management. As a Case Manager, you will play a vital role in providing high-quality care to our clients, ensuring their needs are met, and their lives are improved.

Key Responsibilities:
  • Conduct in-home nursing visits for clients, assessing their physical and emotional needs.
  • Develop and implement personalized care plans to meet the unique needs of each client.
  • Collaborate with healthcare professionals, family members, and caregivers to ensure seamless care coordination.
  • Provide education and support to clients and their families on disease management, medication adherence, and healthy lifestyle choices.
  • Monitor client progress, identifying areas for improvement and making adjustments to care plans as needed.
Requirements:
  • Graduate of a state-approved school of professional nursing.
  • Current, unrestricted RN license in the state(s) of practice.
  • Minimum of two (2) years nursing experience, including one (1) year in home care or a closely related field.
  • One (1) year of supervisory and/or case management experience preferred.
