Responsibilities- Provide care to residents in a variety of settings, including long-term care facilities and community-based settings.
- Assess residents' physical and psychosocial needs and develop individualized care plans.
- Administer medications and treatments as ordered, and provide technical support to healthcare professionals as needed.
- Document resident interactions and outcomes related to resident care, medication, behavior, activity, and dietary.
- Collaborate with interdisciplinary care teams to ensure high-quality care and services.
- Graduate from an accredited practical nursing program as a Licensed Practical Nurse.
- Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges.
- Completes required annual continuing education units (CEU) and competency training requirements of the department and facility.
