Job Summary

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team of dedicated healthcare professionals. As an LPN, you will provide high-quality patient care in a variety of settings, including home health, hospice, residential, and private duty.

Responsibilities
  • Provide individualized care to clients, following their personalized plan of care and promoting their dignity and independence.
  • Monitor clients' vital signs and report any abnormalities to the case manager and physician.
  • Administer medications, feedings, oxygen, and ostomy care as needed, and document the results of these interventions.
  • Collaborate with other healthcare professionals, such as physical therapists, occupational therapists, and speech therapists, to ensure comprehensive care.
  • Implement and maintain infection control policies, reporting any possible communicable diseases or infections.
  • Accurately document all nursing actions, communication with family and healthcare team, and progress towards established goals.
  • Order supplies, medications, and feedings as needed to ensure adequate stock.
  • Ensure all patient needs are met in accordance with physician instructions.
  • Monitor incidents and complete incident reports in a timely and accurate manner.
  • Maintain and report faulty medical equipment.
Qualifications
  • Current license as an LPN or LVN in good standing from the state(s) of practice.
  • Current Cardiopulmonary resuscitation certification.
  • Ability to work in a home-like setting.
  • Physical ability to bend, stoop, push, pull, sit, and walk for periods of time.
  • Able to utilize proper body mechanics while lifting 50 pounds.
  • Knowledge of individual service plans, including understanding of client rights and health needs.
