As a Social Worker, you will play a vital role in supporting the psychosocial needs of our residents, advocating for their rights and dignity, and collaborating with our interdisciplinary care team to deliver exceptional care.
Key Responsibilities- Conduct comprehensive psychosocial assessments and develop individualized care plans to address the unique needs of each resident.
- Facilitate care conferences and coordinate transitions, including room changes, discharge plans, and end-of-life preferences.
- Provide emotional support and advocacy for residents and their families, ensuring their voices are heard and their needs are met.
- Collaborate with the care team to develop and implement behavior plans, safety agreements, and hospice referrals.
- Coordinate pre-admission counseling, intake processes, and admission paperwork, ensuring a smooth transition for new residents.
- Build and maintain relationships with referral sources, community providers, and families to promote our services and support the needs of our residents.
- Bachelor's Degree in Social Work (BSW) from an accredited school of social work.
- Licensed by the State of Minnesota Board of Social Work.
- 0-1 year of experience in social work, preferably in long-term care or transitional care settings.
