Support Specialist for Community Living

The ideal candidate for the Support Specialist role will be available to work primarily during after-school hours. There is flexibility regarding the number of clients served, with each individual typically receiving 1-3 hours of support weekly. Community Living Supports aim to enhance or sustain personal independence, aiding individuals in achieving their goals related to community involvement, autonomy, and productivity.
Key Responsibilities:
- Assisting with meal preparation
- Facilitating independent living skills
- Supporting activities of daily living, including personal hygiene and daily routines
- Enhancing social skills
- Promoting health and safety
- Providing guidance in anger management
- Encouraging self-esteem development
The candidate must possess experience working with children, have the capability to transport them using their own vehicle, and be able to create a weekly schedule while planning community activities. Additionally, the worker is expected to provide respite care for children with significant emotional challenges at least once a month on Saturdays. There are two available shifts: morning (9:00 AM to 12:30 PM) and afternoon (1:00 PM to 4:30 PM).
Additional Duties:
- Maintaining ongoing communication with the Community Living Support Supervisor
- Participating in at least one Saturday group respite session monthly
- Collaborating with therapists as needed
- Independently coordinating schedules with families to meet the required hours of service
- Documenting face-to-face interactions with clients within 48 hours
- Attending monthly group supervision meetings
Family Outreach Center, Inc. is dedicated to providing essential mental health services, treatment for substance use disorders, mental health support within the jail system, in-home and school-based services for children, family support, educational programs, and violence prevention initiatives to those in need.
