Job Summary

Clinical pharmacists at Monument Health play a vital role in delivering comprehensive medication management that optimizes patient outcomes. Working in collaboration with other providers, they coordinate care among providers and caregivers, ensuring seamless transitions across various settings in accordance with hospital standards, policies, and federal and state pharmacy laws and regulations.

Key Responsibilities
  • Identify medication-related problems, including appropriateness of drug, dose, route, frequency, and duration, and minimize therapeutic duplication.
  • Monitor patient response to therapy and provide accurate and adequate drug information to multi-disciplinary team members and patients and their families.
  • Follow and manage pharmacy protocols, such as heparin, warfarin, pharmacokinetic, IV to PO, therapeutic interchange, and renal dosing.
  • Participate in quality improvement projects and lead development, implementation, and maintenance of pharmacy services in a specialty area.
  • Act as a resource in a specific area of specialty and consistently document consults, pharmacist interventions, adverse drug reactions, and medication variances relative to peers.
Requirements
  • Education: Bachelor's in Pharmacy
  • Certifications: Pharmacist - South Dakota Board of Pharmacy, Advanced Cardiac Life Support (ACLS) Certification - American Heart Association, Basic Life Support (BLS) Certification - American Heart Association
  • Preferred: Education - Doctorate in Pharmacy, Certifications - Post-graduate Year 1 (PGY-1) - American College of Clinical Pharmacy, Pediatric Advanced Life Support (PALS) Certification - American Heart Association, Work Experience - 3+ years Hospital Experience, 3+ years Clinical Experience, 1+ years Pharmaceutical Experience
Work Environment

Light work - Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects.
