Optum Home Infusion Nurse Job Description

At Optum, we're dedicated to delivering care and improving health outcomes for millions of people worldwide. As a Home Infusion Nurse, you'll play a critical role in our mission by providing clinical care and education to patients receiving parenteral medications in the comfort of their own homes.

Key Responsibilities:

  • Establish and maintain positive relationships with patients and their families, providing education on self-care and self-administration of infusion therapy
  • Assess patient appropriateness for home infusion therapy, including safety of the home environment and availability of caregiver support
  • Obtain and document patient medical history, including diagnosis and treatment plans
  • Complete all required clinical documentation thoroughly and timely, including nursing plans of care and visit notes
  • Participate in case conferences to comprehensively plan patient care with the multidisciplinary care team
  • Provide skilled care to patients of varying ages and medical conditions, as outlined in the plan of care and in accordance with physician orders

Requirements:

  • Active and unrestricted licensure as a Registered Nurse (RN) in the state of SD
  • Current Basic Life Support (BLS) and/or CPR certification, or willingness to obtain
  • 1+ years of experience working as an RN in a clinical care setting that contributes to proficiency in infusion therapy
  • Proficient with electronic medical record documentation
  • Willing and able to work with an interdisciplinary team of professionals, including pharmacists and technicians, physicians and other licensed prescribers, registered dietitians, home health agencies, and others as needed to provide comprehensive care and support of patients and caregivers

Preferred Qualifications:

  • Experience administering infusion therapy in a home or alternate-site setting
  • Knowledge of and experience with Microsoft Office and electronic medical records (EMR) programs
  • Knowledge of the Infusion Nursing Society (INS) standards of practice, policies and procedures
