Clinical Responsibilities

The Hospice On-Call RN will assess the impact of terminal illness on patients' physical, functional, psychosocial, and environmental needs, documenting this information in clinical records and case conferences.

You will also apply specific criteria for admission and re-certification to hospice care to establish appropriate levels of care and patients' eligibility.

In addition, you will implement and develop plans of care, educating patients and families about disease processes, self-care techniques, end-of-life care, nutrition, and dietary needs.
