Home Health Care Registered Nurse Job Opportunity

Montefiore Medical Center is seeking a skilled Registered Nurse (RN) to join our Home Health team in Edgewater, New Jersey.

Job Summary

We are looking for a compassionate and experienced RN to provide high-quality patient care in the comfort of patients' homes. As a Home Health Care Registered Nurse, you will be responsible for assessing patients' needs, developing and implementing individualized care plans, and collaborating with healthcare teams to ensure seamless care transitions.

Responsibilities
  • Assess patients' physical, emotional, and social needs
  • Develop and implement individualized care plans to meet patients' needs
  • Provide skilled nursing care, including wound care, medication management, and education
  • Collaborate with healthcare teams to ensure seamless care transitions
  • Document patient progress and outcomes
Requirements
  • Current RN licensure in New York State
  • 2 years of experience in home health care
  • Knowledge of Medicare conditions of participation
  • Ability to educate and motivate staff
