Home Health Care Registered Nurse Job Opportunity

Montefiore Medical Center is seeking a skilled Registered Nurse (RN) to join our Home Health team in Edgewater, New Jersey.

Job Summary

We are looking for a dedicated and compassionate Registered Nurse to provide high-quality patient care in the comfort of their own homes. As a Home Health RN, you will be responsible for assessing patient needs, developing and implementing individualized care plans, and collaborating with our interdisciplinary team to ensure seamless care transitions.

Responsibilities
  • Provide comprehensive nursing care to patients in their homes, including wound care, medication management, and education on disease management and self-care.
  • Conduct thorough assessments of patients' physical, emotional, and social needs, and develop personalized care plans to address these needs.
  • Collaborate with our interdisciplinary team, including physicians, therapists, and social workers, to ensure comprehensive care and effective communication.
  • Document patient progress, including assessments, care plans, and outcomes, in a timely and accurate manner.
  • Participate in ongoing education and training to stay current on best practices in home health care and to enhance your clinical skills.
Requirements
  • Current RN licensure in New York State.
  • 2 years of experience in home health care or a related field.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Strong assessment and problem-solving skills.
