Job Summary

We are seeking a skilled Physical Therapist to join our team at Foundation Senior Services Inc. as a Home Health Therapist. In this role, you will be responsible for implementing individualized home health therapy programs that encompass a broad spectrum of responsibilities including assessment, treatment planning, and patient education.

Key Responsibilities:

  • Develop and implement patient-centric care plans based on comprehensive assessments and including the involvement of significant others where appropriate.
  • Deliver direct patient care, focusing on evaluating functional needs and outcomes, while collaborating with other specialists to adjust care plans as necessary.
  • Lead in the initiation of skilled care episodes, focusing on potential developmental assessments (specific to PT role).
  • Maintain high standards of therapy practice within established norms of care.
