Role Overview
The Registered Nurse (RN) in Case Management is responsible for delivering patient-centered care by coordinating the patient's plan of care, managing their length of stay, and ensuring a safe discharge plan in collaboration with a multidisciplinary team.
Key Responsibilities
The RN CM will:
- Identify patients at risk for adverse outcomes during transitions of care.
- Conduct comprehensive assessments of patients' psychosocial, medical, and discharge needs.
- Reassess clinical conditions and develop strategies to mitigate readmission risks.
- Collaborate with healthcare teams to coordinate care and facilitate successful transitions.
- Engage with patients and families to establish realistic goals and expectations.
- Utilize evidence-based tools to evaluate care progression and escalate issues as necessary.
- Document care coordination activities and maintain effective communication with all stakeholders.
- Focus on cost control while ensuring quality and safety in patient care.
- Participate in performance improvement initiatives to enhance patient outcomes.
Qualifications
To be considered for this role, candidates should possess:
- An Associate Degree in Nursing or Nursing Diploma (Bachelor's Degree preferred).
- A minimum of 2 years of experience in case management or 3 years in clinical nursing.
- Experience with InterQual criteria.
