Job Title: Team Leader

Job Summary:

The Nathaniel ACT Team Leader is responsible for directing and supervising ACT staff activities, leading organizational, clinical, and service planning meetings, and coordinating administrative functions. In collaboration with the ACT team prescriber, the Team Leader provides clinical direction to staff. The Team Leader works closely with the CASES ATI Intake Assessment Team to identify and screen individuals who are eligible and appropriate for the services provided by the team.

Key Responsibilities:

  • Oversee the clinical and administrative operations of the Nathaniel ACT Team, including daily team meetings, weekly clinical meetings, timeliness and quality of clinical documentations and reporting, intakes and discharges, team's after-hour emergency line, use of service dollars, and team members' schedules.
  • Ensure the team delivers culturally and developmentally appropriate, trauma-informed, recovery-oriented services and effectively applies motivational interviewing.
  • Provide regular clinical supervision, training, and professional development opportunities for team members.
  • Provide direct clinical care and services to consumers in the community when needed.
  • Ensure the team meets its performance outcomes and operational standards as defined by New York State Office of Mental Health (OMH) licensing standards and New York City Department of Health and Mental Hygiene (DOHMH) contracting standards.
  • Maintain working relationships with partners within the criminal legal system, including court representatives, attorneys, judges, and the Mayor's Office of Criminal Justice (MOCJ).
  • Oversee team's incident reporting procedures according to OMH standards and ensure team adheres to the Incident Review Committee (IRC) policies and procedures.
  • In collaboration with the Quality Assurance department, conduct utilization reviews and quality assurance reviews to ensure that the team operates in compliance with New York State OMH guidelines and regulations, New York City DOHMH operating standards, as well as HIPAA confidentiality regulations.
  • Consult with senior clinical leadership on complex, high-risk cases, as well as team and personnel issues.
  • Coordinate monthly Medicaid billing using electronic medical records to select billable progress notes and ensure team achieves billing targets defined by program budget.

Requirements:

  • LMSW, LCSW, RN, LMHC, or Licensed Psychologist (PsyD/PhD).
  • Two years of supervisory experience.
  • At least three years of experience providing services to adults with serious mental illness.
  • Experience working in community mental health settings that provides services to recipients in-vivo in the community.
  • At least two years of experience working on a multidisciplinary team.
  • Experience working with and knowledge of the needs and challenges faced by individuals involved in the criminal legal system.

Preferred but not required:

  • Fluency in Spanish.
