Palliative Care Nurse Practitioner Opportunity

Mercy Health-Lourdes Hospital is seeking a skilled Palliative Care Nurse Practitioner to join our team in a full-time employment opportunity. The successful candidate will be responsible for providing palliative services in a non-acute setting, working towards reducing ED visits, acute care hospitalization, and healthcare costs, while enhancing patient and family experience of care ratings.

Key Responsibilities:

  • Deliver high-quality palliative care services to patients in a community setting
  • Collaborate with healthcare teams to develop and implement patient-centered care plans
  • Conduct regular patient assessments and adjust care plans as needed
  • Provide education and support to patients and families on palliative care and end-of-life options
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
