Responsibilities- Assess patients' physical and emotional needs, develop and implement individualized care plans, and provide education and support to patients and their families.
- Collaborate with the interdisciplinary team, including physicians, nurses, social workers, and other healthcare professionals, to ensure comprehensive and coordinated care.
- Provide routine and emergency assessments, recertification visits, and other necessary interventions to meet patients' needs.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination and transitions.
- Participate in ongoing education and professional development to stay current with best practices and evidence-based guidelines in palliative and hospice care.
- Master's degree in Nursing required; CNS or NP license in state of employment required.
- Certified as a Nurse Practitioner in Adult, Family, Acute, or Geriatric required.
- Active NPI number and DEA certification if state allows prescription of controlled substances by APPs.
- Board Certification in Hospice and Palliative Care preferred but not required.
This role requires regular standing, walking, and manual dexterity, as well as the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. Exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected.
