Job Summary

We are seeking a highly skilled and compassionate Primary Care Provider to join our team at HealthPoint. As a Primary Care Provider, you will be responsible for providing comprehensive primary care services to patients of all ages in a primary medical care ambulatory clinic setting and in Health care for the Homeless facilities.

Key Responsibilities
  • Provide comprehensive primary care services to patients of all ages, including routine check-ups, health maintenance, and disease prevention.
  • Assess patient needs for specialty referrals, diagnostic and in-patient care, and make appropriate referrals for service.
  • Provide patients and family members with counseling and education regarding health maintenance, disease prevention, diagnosis, treatment, and need for follow up as appropriate during each patient visit.
  • Complete accurate and timely documentation in patient records and superbills.
  • Participate in patient triage system and shared call system with other providers.
  • Perform minor outpatient procedures in accordance with the scope of practice and HealthPoint privileging policy.
  • Administer medications in clinic as needed.
  • Assist with meeting clinic patient care team productivity and quality care standards.
  • Participate in development and utilization of HealthPoint practice guidelines and procedures.
  • Participate in audits, and meet standards of HealthPoint Quality Improvement, Risk Management and Utilization Management programs.
Requirements
  • Graduate from an accredited nurse practitioner or physician assistant program.
  • Washington State nurse practitioner or physician assistant license. Board certification required.
  • Demonstrated operating knowledge of computers: intermediate level ability with Word, Outlook, and other required software programs; prior experience with electronic medical records preferred.
