Job Summary

We are seeking a skilled Care Coordinator to join our team at Dartmouth Health. As a key member of our healthcare team, you will play a vital role in ensuring the highest quality patient care and experience.

Key Responsibilities
  1. Triage and Coordination: Facilitate the development of a plan of care in collaboration with patients, families, caregivers, and healthcare teams. Identify patient needs and preferences, and coordinate interdisciplinary treatment and care plans.
  2. Care Progression: Monitor patient progress, advance care plans to achieve desired outcomes, and alert healthcare teams to potential revisions in response to changes in patient needs and condition.
  3. Communication and Advocacy: Communicate effectively with patients, families, caregivers, and healthcare teams. Advocate on behalf of patients and families for service access and insurance benefits within regulatory constraints.
  4. Quality Improvement: Evaluate patient outcomes affected by the care management process and identify opportunities for improvement. Participate in quality improvement initiatives within the clinical setting or other organizational committees.
Requirements
  • Graduate from an accredited Nursing Program required.
  • Bachelor's degree (or matriculated into a Bachelor's degree program with completion within a specified timeframe) with 3 years of clinical experience required.
  • Recent acute care hospital experience preferred.
  • Desired qualities and behaviors include: collaborative team player with strong communication skills, use of a systems approach in planning, problem solving, and decision making, creativity, innovation, risk taking, autonomy, flexibility, receptiveness to change, and a commitment to professional growth.
