Job Summary


We are seeking a compassionate and skilled LPN to join our team. As an LPN with Team Select Home Care, you will have the opportunity to work with pediatric and adult patients with various medical needs, including those with g-tubes, trachs, and ventilators. Your day will be filled with administering medications, charting progress, and communicating with other healthcare providers to ensure our patients receive the best possible care.


Responsibilities



  • Deliver high-quality patient care, including medication administration, ADL assistance, and assessments
  • Work with pediatric and adult patients with various medical needs, including those with g-tubes, trachs, and ventilators
  • Utilize electronic documentation to record patient progress and communicate with other healthcare providers
  • Participate in ongoing education and training to stay up-to-date on the latest home health care practices and procedures
  • Collaborate with our interdisciplinary team to develop and implement individualized care plans for our patients

Qualifications



  • Current MO/Compact State License for LPN or RN
  • Maintain current CPR certification and BLS (Basic Life Support) certification (preferred)
  • Ability to work effectively in a fast-paced, dynamic environment
  • Strong communication and interpersonal skills, with the ability to work collaboratively with other healthcare providers and patients
