Transcend Hospice - Position Overview:
The primary responsibilities include delivering expert nursing care to clients across various age groups within their residences, collaborating with the interdisciplinary team, patients, families, and referral sources; while taking charge of care coordination.

Qualifications:
Educational Background:
Completion of an accredited Diploma, Associate, or Bachelor’s Degree in Nursing

Licensure Requirements:
Must hold a current state license as a Registered Nurse and possess a valid State Driver's License

Professional Experience:
A minimum of two years of experience as a Registered Nurse in a clinical environment is preferred, with a focus on hospice or home health care being advantageous.

Required Skills:
Proficient nursing skills as per established standards of practice. Strong interpersonal abilities and effective communication skills are essential. Documentation of Hepatitis consent/declination is required. Specialized expertise in palliative care and end-of-life support is preferred.

Transportation:
Must have reliable transportation and valid auto liability insurance.

Work Environment:


Work is conducted in patients' homes under varying conditions; potential exposure to blood, bodily fluids, and infectious diseases; flexibility in scheduling is necessary; local travel is required; some exposure to adverse weather conditions; on-call duties may be necessary.


Physical and Mental Demands:
Prolonged periods of sitting and walking are required, with the ability to lift up to 50 lbs. and assist patients.

Work may involve stressful situations requiring timely decision-making and resource management to address patient and family needs.

Hand-eye coordination and manual dexterity are essential.

Core Responsibilities:


Conduct initial and ongoing comprehensive evaluations of the effects of terminal illnesses on patients' physical, functional, psychosocial, and environmental needs, including activities of daily living (ADLs).

Implement and adjust the individualized Plan of Care (POC) as necessary.

Provide consultation and education to patients and families regarding disease processes, nutritional needs, self-care strategies, and end-of-life care. Offer training to other staff members as required.

Initiate appropriate preventive and rehabilitative nursing interventions.

Document clinical and progress notes that reflect advancements toward established objectives.

Coordinate all patient and family services, prioritizing needs in collaboration with the interdisciplinary team.

Utilize a case management approach and refer to additional services as needed.

Notify physicians and other relevant personnel of any changes in patient needs and the outcomes of interventions, while assessing patient and family responses to care.

Determine the scope and frequency of services required based on patient acuity and family needs.

Evaluate the caregiver's capacity to meet the patient's immediate needs upon admission and throughout the care process.

Assess personal support needs and utilize available resources to address them.

Communicate information effectively using the appropriate processes and technologies available within the organization.

Apply specific criteria for admission and re-certification to hospice care to establish suitable levels of care and determine patient eligibility.

Perform clinical tasks for patients as necessary to adhere to their POC.

Fulfill on-call responsibilities and provide on-call services to patients and families as assigned.
