We are seeking a skilled Physical Therapist to join our team at Optage Homecare, a division of Presbyterian Homes & Services. As a Physical Therapist, you will play a vital role in providing high-quality care and services to our homecare clients in the East metro and western Wisconsin areas.
Responsibilities- Plan, develop, and implement individualized physical therapy care and services to residents, consistent with regulations and established rehabilitation policies and procedures.
- Provide direct care to residents with physician-ordered evaluations and treatments, based on regulations and established rehabilitative procedures and best practices.
- Participate in patient/resident care planning with the interdisciplinary team and other healthcare providers.
- Provide patient/resident and family education/consultation, and communicate resident status and needs to the patient/resident, their family, staff, and other professionals.
- Maintain accurate records and documentation, in accordance with practice standards, third-party payers, and regulatory agencies.
- Graduate from an accredited Physical Therapy program.
- Current licensure in the state of practice, as required.
- Competence in physical therapy evaluation and treatment.
- Demonstrated compatibility with PHS's mission and operating philosophies.
- Demonstrated ability to read, write, speak, and understand the English language to communicate with all customers.
