Senior Case Manager Job Description

We are seeking a highly skilled and experienced Senior Case Manager to join our team at Professional Case Management. As a Senior Case Manager, you will be responsible for providing direct care and case management to assigned clients, collaborating with regional directors and other personnel to identify and correct issues, and implementing and evaluating care provided.

Key Responsibilities:
  • Provide direct care and case management to assigned clients
  • Collaborate with regional directors and other personnel to identify and correct issues
  • Implement and evaluate care provided
  • Participate in client care conferences as needed
  • Serve as a local on-site clinical resource as needed
  • Assist with staffing coordinators regarding staffing and scheduling of personnel
  • Adhere to nursing delegation guidelines
  • Ensure adherence to Agency policies
Requirements:
  • Graduate of a state-approved school of professional registered nursing
  • Current, unrestricted RN license in the state(s) of practice
  • Minimum of two (2) years nursing experience, including one (1) year in home care or closely related field
  • One (1) year of supervisory and/or case management experience preferred
  • Current CPR certification
What We Offer:
  • $44.00 Hourly plus shift differentials, where applicable
  • Bonus Eligible
