About the Role

  • This is a senior-level position that requires experience and expertise in case management and supervision.
  • You will be responsible for leading a team of RN Case Managers and overseeing the provision of care to clients.

Key Responsibilities

  1. Leadership and Supervision
  • Direct assigned team members of RN Case Managers in the provision of care in accordance with Agency policy and state-specific nurse practice act.
Care Planning and Coordination
  • Plan, implement, and evaluate care provided to clients.
  • Participate, coordinate, and manage client care conferences as needed.
Client Support
  • Serve as a local on-site clinical resource as needed and provides support to ensure client's home care needs are met.
Quality Assurance and Improvement
  • Assist and collaborate with the regional director and other personnel to identify and correct issues and/or improve services.

Requirements and Qualifications

  • A graduate of a state-approved school of professional registered nursing.
  • A minimum of two years of nursing experience, including one year in home care or a closely related field.
  • One year of supervisory and/or case management experience preferred.
  • Current CPR certification.

Benefits and Compensation

  • An estimated hourly rate of $44.00, plus shift differentials where applicable.
  • Bonus eligible.
